Breast augmentation is a popular aesthetic choice for many women today. Pursuing a better figure costs not only money and time but also aftercare after surgery! The whole process cannot be skipped or done carelessly. Today we will teach you how to actually perform "post-surgery massage," how long the schedule must be maintained, and the particulars of the weekly massage techniques, along with basic patient education and a Q&A on aftercare. You can find all the answers in this article!

What Is Breast Augmentation Massage, and How Long Should You Do It?

First, why is massage needed after surgery? Because after breast augmentation the implant pocket needs to be "appropriately expanded," and the tissue around the implant begins to form a capsule around it, so without massage the breast gradually becomes hard and may even become deformed, which is what is known as "capsular contracture." We recommend making the most of the golden first three months, during which moderate massage is needed (because constitutions differ, even if the implant itself needs no massage, you should still follow your doctor's instructions!). This lowers the chance of capsular contracture. Massage is generally needed for about six months, but the actual time varies with the implant material and individual circumstances!

Massage Method for the First Three Days After Surgery

Although breast massage is needed after surgery, during the first three days avoid sleeping on your stomach and avoid massage. Massaging right after surgery may cause bleeding!

Massage method from day 3 to the end of week 1:

Another reminder! Be sure to wait until three full days after surgery before massaging!

1. Basic Inward-Push Massage (for normal or outward-set breast shapes)

Do the two-handed inward-push motion for 2 minutes each time, once every 30 minutes to 1 hour, at least 4 to 5 times a day.

First cross your arms over your chest with your fingertips touching the bottom of the implants. Use the front of your fingers to move the implants toward the center until a clearly defined line of cleavage forms between the breasts. (If you can't keep going for 2 minutes, you can do it in segments, as long as the total reaches 2 minutes.)

2. Basic Outward-Push Massage (for breasts set closer together)

Do the outward-push motion for 1 minute per side each time, once every 30 minutes to 1 hour, at least 4 to 5 times a day.

Use the palm on the same side (right hand for the right breast, left hand for the left breast) to push the implant outward from its inner edge. Do not let go while pushing outward, and add your other palm to help push; the palm on the same side must press firmly against the ribs. (If you have a dominant hand or posture issue, you can switch hands: use the opposite palm to push the implant from the inside outward, with the other palm assisting.)

Massage Method From One Week to One Month After Surgery

After doing the inward/outward push first, begin the "cross-shaped massage," about 10 to 15 minutes per side, at least three times a day.

After 7 days, add the cross-shaped massage movements, pushing the implant up, down, inward, and outward, 10 minutes per side. You should feel the implant sliding within a defined space for the massage to be effective.

Because breast shapes differ, the push directions also differ, so follow your doctor's instructions. Starting on day 7, first do the basic massage: 2 minutes of inward push, or 1 minute of outward push on each side, repeat twice, and then begin the cross-shaped massage, 10 minutes per side. (You do not need to be topless for the cross-shaped massage. You can keep your bra on and just pull the chest band down to your abdomen.)

How to Do the Cross-Shaped Massage

Always use the opposite hand when massaging, so the pectoralis major can relax. If one hand lacks strength, you can use both hands to assist, but for the inward push, to avoid incorrect posture, we recommend still using one hand.

Massage Method From One Month to Three Months After Surgery

Do each of the two movements for 10 minutes per side: first the eight-direction massage, then 10 minutes of the star-pattern massage, at least three times a day.

One month after surgery, the tissue around the implant has begun to form a capsule, so the basic massage is no longer needed, and you should switch to star-
pattern massage. Because everyone's breast shape is different, the push directions also differ, so be sure to follow your doctor's instructions.

Why Is Massage Needed After Breast Augmentation?

How to Do the Star-Pattern Massage

  • Use the opposite palm to push the implant outward and hold for 10 seconds.
  • First use the base of the thumb of the opposite palm to push toward the upper outside and hold for 10 seconds. You can use both hands to assist.
  • Use the base of the thumb of the opposite palm to push straight up and hold for 10 seconds.
  • Use the base of the thumb of the opposite palm to push diagonally up from outside to inside and hold for 10 seconds.
  • The four fingers of the opposite palm are held upright at a right angle; use the fingertips to move the implant from outside to inside, pushing it inward, and hold for 10 seconds.
  • Use the lower edge of the palm to push toward the inside and downward and hold for 10 seconds. You can use both hands to assist.
  • Open your palm facing up, use the lower edge of the palm to push straight down, and hold for 10 seconds. You can use both hands to assist.
  • Use the lower edge of the palm to push toward the outside and downward and hold for 10 seconds.

We recommend attending follow-ups on schedule to track your progress, and adjusting the massage method and frequency as your doctor advises!

How to Use the Chest Band After Breast Augmentation

After breast augmentation, you need to use a chest band for a period of time. At first, its main purpose is to compress the upper chest to reduce bleeding; afterward it prevents the implants from moving upward and helps adjust breast shape. A chest band is generally worn 24 hours a day for 2 months, then only at night while sleeping for another 1 month, up to 3 months in total, but follow your doctor's advice at follow-up visits. If you have sensitive skin, apply essential oil or lotion before putting it on! If the implants sit at different heights, the chest band can also be used to adjust them!

First 3 days after surgery: If you want to wash, wipe yourself with a towel. Do not take off the chest band to clean yourself, as it may not be put back on properly, causing the breasts to shift or even causing excess bleeding at the wound

Days 3 to 10 after surgery: You may temporarily remove the chest band for a simple shower, but until the wound has healed, never use body wash to scrub the wound. Whether to remove or put on the chest band should be decided by your doctor's evaluation, so never take it off on your own to bathe!

1. When One Side Is Too High

Wrap the chest band diagonally across the upper edge of the chest, pulling the band on the higher side downward, folding the band on the normal side inward and pulling it upward, and loosening the bra strap on the higher side.

2. When One Side Is Too Low and the Other Too High

Press the band on the higher side down from the upper edge of the chest, loosen the bra strap on that side, and lift the lower side up from the lower edge of the chest with the band, tightening the bra strap on that side. This is called the S-shaped chest band wrap.

Recovery Education and Q&A Guide

Q: Which breast augmentation methods need no massage after surgery?

A: Textured and rough-surface implants, fat transfer breast augmentation, and some teardrop implants may not need massage, whereas "smooth" implants require diligent massage after surgery. Whichever material you choose, we recommend confirming with your doctor before surgery.

Q: The clinic nurse massages me. Do I still need to massage myself at home?

A: Massage is needed frequently after breast augmentation, so one visit to the clinic a day is not enough. We recommend massaging at home every half hour to one hour, which not only lowers the chance of capsular contracture but also keeps the breasts soft.

Q: What if the implants are at different heights after surgery?

A: You can adjust with a chest band. For example, pull the band down on the higher side so its force pushes the implant down, and remember to loosen the strap at the same time. For an implant sitting too low, use the band to lift it up while tightening the bra strap.

Q: Can I do weight training after breast augmentation?

A: About two weeks after surgery, you can begin light exercise and lower-body weight training, and after one month you can begin more vigorous exercise and upper-body weight training. The exact timing varies with each person's condition, so we recommend regular follow-ups and confirming with your doctor before starting.

Q: Can I squeeze my breasts after breast augmentation?

We recommend handling them gently for the first month, and being careful during sexual activity as well.

Q: Should I wear an underwire or wire-free bra after breast augmentation?

A: We recommend not wearing an underwire bra during the first month after surgery, to avoid deforming the breasts. From the third month, once your doctor confirms, you can wear a soft underwire bra to help maintain breast shape. Once you start wearing underwire bras, the guideline is appropriate support for the breasts. Do not wear styles that overemphasize pushing together and lifting, and choose a moderate cup size. A cleavage gap of about 1 to 1.5 cm is ideal; you don't need to squeeze it into a single line, but make sure the upper chest feels supported. Bras with wider straps and better support are best.

Q: Ice or heat after breast augmentation?

The first three days after surgery are the swelling and pain period, and you can apply ice to reduce bleeding and lower swelling and pain. After three days you can begin continuous warm compresses, which promote blood circulation and metabolism to help reduce swelling and ease pain.

Q: How should I sleep after breast augmentation?

Prop yourself up with extra pillows. Sleeping on your back is best. Try not to lie on one side for long periods, as this easily compresses the breasts and risks displacement, and sleeping on your stomach adds to the burden on the implants.

Q: What is the wall-climbing exercise after breast augmentation?

If your incision is in the armpit, you may have trouble raising your arms. After the stitches are removed, we recommend the "wall-climbing hand exercise" to stretch the armpit incision and the tight areas, which helps you return to normal activity sooner!

  • Stand 20 cm from the wall
  • Place your palm on the wall
  • Walk your fingers up the wall until you feel tightness
  • Repeat the movement 5 to 7 times
